SUMMARY: Being a case manager on the Assertive Community Treatment Team (ACT) provides a unique opportunity to work as part of a multidisciplinary team providing services to adults who have serious mental illness. Psychiatrists, nurses, and case managers work as a team to provide intensive services in clients' homes and in the community to prevent hospitalization and maximize community integration. Community integration includes the ability to have a job, friends, and safe, affordable housing. Case managers have specific skills and training to assists clients to achieve these goals.
DUTIES: 1) accepts assigned referrals and completes functional assessments, treatment plans, and other necessary paperwork to comply with treatment and funder needs; 2) coordinates and provides client services in areas of mental health, housing, employment, and other basic needs; 3) works cooperatively with other service providers and coworkers; 4) completes progress notes and other documentation as required.
5) Other duties include: Rotating weekend and holiday coverage and on-call schedule. On call shifts approximately 3-5 times per month.
MINIMUM QUALIFICATIONS: BA degree in human service degree and 2000 hours experience providing mental health services. Driver's license, ability to pass a background/DMV check and ability to transport clients in own vehicle. The ACT team services are available 7:30 am to 6 pm 5 days a week with 6-hour coverage on Saturdays and Sundays (rotating schedule approximately every 2 months); Holiday and on-call services are provided with a staff rotation.
DESIRED QUALIFICATIONS: Experience with serving clients with dual diagnosis (mental illness, chemical dependency). Cultural competence serving persons of non-dominant culture.
