SUMMARY:  Homeless Case Management staff are responsible for identifying homeless adults in Carlton County through direct street outreach efforts in the community. The primary goal of homeless outreach is to build rapport with unsheltered individuals who may be experiencing serious mental illness, provide advocacy, and assist in connecting to community resources to ensure the person's basic needs are met.


DUTIES:

-  The Homeless Case Manager will accept referrals for homeless individuals, as well at those at risk of homelessness, from local hospitals, law enforcement, and social services agencies to assist in navigating the process to find appropriate housing that meets the person's needs.

-  Responsible for providing Housing Support Services to seven (7) tenants at White Pine Apartments, as well as Housing Sustainability and Support to five (5) tenants at the Outreach Center Apartments.

-  Demonstrate professionalism and establish excellent working relationships with community partners (Carlton County Health & Human Services, Cloquet HRA, United Way, Lakes & Pines CAC, Fond du Lac Reservation), law enforcement, and local landlords.

-  Receive training in HMIS (Homeless Management Information System) and complete coordinated entry assessments to assist homeless individuals in finding appropriate housing.

-  Required to complete any other documentation (i.e. housing service plan) related to obtaining and maintaining housing for homeless adults.

-  In addition to housing, the Homeless Case Manager will be responsible for managing a partial ARMHS (Adult Rehabilitative Mental Health Services) caseload as well as any other duties as assigned.

-  The Homeless Case Manager works in a team setting and will be required to participate in multidisciplinary team meetings and clinical supervision as scheduled.


MINIMUM QUALIFICATIONS: 

-  B.A. degree in human services plus 2000 hours of supervised experience (1-year) in adult and/or children's mental health setting OR

o  Completed master's degree in human services or behavioral science. OR

o  Meet Case Manager Associate requirements.
