Coordinated Services Team Facilitator

Summary: Provide family-based services as outlined in Wisconsin Statutes 46.46 to children and families by leading the Coordinated Services Team (CST). Act as lead in developing policies and procedures, building an effective collaborative team, and developing plans of care for each client that outline responsibilities of each team member, a timeline, and outcome expectations.
Duties: Ensure completion of quarterly reports, including the collection and recording of data, to evaluate the effectiveness of care. Ensure appropriate disposition of referrals, determine eligibility, and monitor services. Provide community-based case management service; outreach, aftercare, follow up; and professional consultation and communication education as requested.
Minimum Qualification: A Bachelor's degree is required with preference for social work, psychology or other closely-related field. Vehicle required; Mileage reimbursement available.
Desired Qualifications: 5 years experience in the Human Service Field. Strong organizational skills, knowledge of computers, experience working in and on professional committees.
