Provide nursing care to clients with severe & persistent mental illness (SPMI) in collaboration with other members of the Transitional Assertive Community Treatment (T-ACT) team.
Mental Health Practitioner provides direct support services, including skills training, crisis assistance, treatment plan development, medication administration, group facilitation, and one on one support/interventions, and discharge planning. The Certified Peer Specialist (Peer Specialist) will provide behavioral health services that foster supportive peer relationships with clients in the various community support programs. Certified peer specialists are individuals who have had personal experience of mental illness and are trained to be direct service mental health staff. Applicants must have completed the DHS State approved training and be certified as a Peer Specialist.
