The Outreach Center Cook will prepare and serve community meals at the Cloquet Outreach Center (ORC) multiple days a week. This role collaborates with HDC staff and ORC volunteers to plan menus, manage inventory, and support community engagement. The ideal candidate is organized, community-focused, and committed to food safety.

Provide nursing care to clients with severe & persistent mental illness (SPMI) in collaboration with other members of the Transitional Assertive Community Treatment (T-ACT) team.

Mental Health Practitioner provides direct support services, including skills training, crisis assistance, treatment plan development, medication administration, group facilitation, and one on one support/interventions, and discharge planning. The provider will work collaborate with the treatment team to collaborate on client care, progress on treatment goals, and referrals for ongoing support and mental health services.

The successful candidate will work with a multi-disciplinary team of internal and external providers to assess needs and apply interventions appropriately.

The Certified Peer Specialist (Peer Specialist) will provide behavioral health services that foster supportive peer relationships with clients in the various community support programs. Certified peer specialists are individuals who have had personal experience of mental illness and are trained to be direct service mental health staff. Applicants must have completed the DHS State approved training and be certified as a Peer Specialist.
