SUMMARY: Provide nursing care to clients with severe mental illness (SMI) in collaboration with other members of the Assertive Community Treatment (ACT) team.

DUTIES: Essential duties and responsibilities include the following (other duties may be assigned):

-  Provide direct and supportive clinical services as needed both in the office and in the field, including:  telephone triage, obtaining vital signs, refilling prescriptions electronically, coordinating care with other providers, initiating prior authorizations for medications, assisting in referrals to hospital or crisis stabilization facilities as needed, electronic documentation, and special projects as needed.  This position reports to the assigned Advanced Practice Registered Nurse (APRN)/nursing supervisor, and ultimately the Director of Psychiatry.

-  Assist the ACT APRN and/or psychiatrist in the process of evaluation, treatment planning and ongoing treatment of the ACT clients.

-  Provide direct services for the ACT team at HDC including crisis evaluation and triage, which includes assessment of patient's response to medications and side effects, along with patient education. Make judgments about urgency and the need for psychiatric evaluation. Assess patients to detect behavior patterns and reports observations to medical staff as indicated.

-  Provide nursing assessment for each patient in program along with nursing treatment plan.

-  Provide care coordination for both mental and physical health care with patients, families, staff, and others.

-  Documentation for psychiatric clients, prescription renewals, and nursing progress notes. In addition, ACT nurses carry a small caseload and are responsible for completing a bi-annual treatment plan, and functional assessment as a part of case management duties.

-  Consult with the supervising psychiatrist or APRN as needed.

-  Travel is required as most of our services are completed in the field.

-  Provide accessible, customer friendly service.

-  Maintain records in accordance with specified procedures.

-  Initiate and follow through on new ideas/programs.

-  Other clinical and administrative duties as assigned; including duties required per MN State Statutes

-  On call/crisis response service as required per MN State Statutes

MINIMUM QUALIFICATIONS: 

-  A.A. or B.A. degree in Nursing.

-  Minimum of one (1) year of experience as psychiatric nurse.

-  Knowledge of community psychiatric systems and resources.

DESIRED QUALIFICATIONS:

-  Previous Nursing Experience in an Assertive Community Treatment Program

-  Previous work experience in Community Mental Health Clinic as triage nurse
