SUMMARY:
Mental Health Practitioner provides direct support services, including skills training, crisis assistance, treatment plan development, medication administration, group facilitation, and one on one support/interventions, and discharge planning. The provider will work collaborate with the treatment team to collaborate on client care, progress on treatment goals, and referrals for ongoing support and mental health services. MHP's work with a multi-disciplinary team of internal and external providers to assess needs and apply interventions appropriately. This position is based in a short-term residential facility. Shifts consist of overnights.
DUTIES:
- Conduct intake assessments.
- Assist in obtaining necessary assessment data and participate in treatment coordination which may include; completing and updating treatment plans, releases of information, progress notes, and referrals to internal and external providers.
- Coordinate needed client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs.
- Teach individual skills related to coping, relapse prevention, and basic household and living skills.
- Maintain appropriate boundaries in a variety of settings and situations.
- Provide crisis assistance as needed.
- Work cooperatively with other service providers, have a strong understanding of program eligibility requirements, and be informed on referral processes for community support.
- Provide program transportation as needed.
- Assist in discharge planning, coordination of care, and linking clients to ongoing support and mental health services.
MINIMUM QUALIFICATIONS:
- Bachelor's degree in human services or related field and 2,000 hours of mental health experience.
o OR high school diploma and 6,000 hours of mental health experience.
- Valid Driver's License and reliable vehicle with proof of insurance.
- Ability to submit and pass a DMV/Criminal background check.
- Strong interpersonal and cultural competency skills.
DESIRED QUALIFICAITIONS:
- Knowledge of community resources and area providers.
- Effective time management skills, ability to be self-directed and motivated.
- Knowledge of employment-related skills and crisis intervention skills.
- Experience working with adults who have been homeless or at risk for homelessness, have chemical use/abuse issues, and have been involved with the legal system.
- Experience working with diverse populations.
