SUMMARY: Case Managers maintain responsibility coordinating and providing services for adults who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers.
DUTIES: Accept assigned referrals and engages clients to initiate services. The case manager assists in obtaining necessary assessment data, including vital signs and participates in integrated treatment coordinating which may include, treatment plan, releases of information, progress notes, and referrals to internal and external providers. The case manager arranges meetings with client and other providers and coordinates needed client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education and other basic needs. He/she works cooperatively with other service providers and has a good understanding of program eligibility requirements and referral aspects associated with community resources.
-B.A. degree in human services plus 2000 hours of supervised experience (1-year) in adult mental health setting OR
- Completed Master's Degree in Human Services or Behavioral Science.
Position also requires; reliable transportation, proof of vehicle insurance, and valid driver's license, and knowledge of resources in St. Louis County.
DESIRED QUALIFICATIONS: Two years of related experience. Experience working with adults who have been homeless or at risk for homelessness, indigent, have chemical use/abuse issues, and have been involved with legal system.
