SUMMARY: Utilize the patient registry to link services, identify and manage care gaps and barriers, facilitate communication among BHH Services team members and other providers, coordinate recommendations for adjustments to treatment, reconcile medications, gather diagnostic results and close the loop on referrals or treatment. Provide information to the consumer about the purpose and services of the BHH, and the consumer's rights and responsibilities.

DUTIES:Establish trusting relationships with clients and their families and engage consumers and identified formal and informal supports in shared decision making. Act as client advocate and liaison, provide information to the consumer about the purpose and services of the BHH, and the consumer's rights and responsibilities. Complete and document initial needs assessment, health wellness assessment and health action plan and update as warranted. Identify consumer's preferences and cultural and spiritual beliefs and integrate into health action plan.  Ensure use of evidence-based practices in services and education.  Assist clients and their families in health literacy through wellness and prevention education, coaching, chronic disease management and navigating complex systems of care. Help identify barriers to achieving optimal health and provide referrals to appropriate services and follow-up through completion to close the loop. Establish a transition plan in partnership with the consumer and consumer's identified supports.  Communicate with other BHH team members to incorporate consumer needs into health action plan. Other duties as assigned in accordance to Human Development Center policy/procedure. 

MINIMUM QUALIFICATIONS:  Bachelor's and 2000 hours (one-year full time employment) experience within a mental health care setting.  Valid Driver's License and reliable vehicle with proof of insurance.  Ability to submit and pass a DMV/Criminal background check.   Interpersonal skills and cultural competency are key in this position. 

DESIRED QUALIFICATIONS:  Bachelor of Social Work and license exam passed or in progress (LSW). Experience in motivational interviewing, crisis prevention, and intervention, and entering information into an Electronic Health Record.  Knowledge of community resources and area providers.  Good time management skills, ability to be self-directed, and disciplined.
